Forest Hill was purchased from the heirs of Adolph Sutro by a private corporation. Groundwork for the neighborhood began in 1912. Forest Hill was designed and actively promoted as a racially segregated, white-only sanctuary for economic elites.
Forest Hill's streets are especially wide since they were initially intended for horse and carriage. Because these Forest Hill roadways did not meet San Francisco's width and grade regulations, they were not initially sanctioned or maintained by the City until 1978. Forest Hill's master plan was created by landscape architect Mark Daniels.
Forest Hills Extension has a population of 1,854 people, with 49 percent males and 51 percent females with a median age of 44. Families with children make up 33% of the population in this neighborhood, while single-female families make up 23%, single-male families make up 12%, and couples make up 64%.

Margaret Sage, the Russell Sage Foundation's founder, purchased 142 acres (57 ha) of land from the Cord Meyer Development Company in 1908. This area was to be utilized for "Forest Hills Gardens," a development on Forest Hills' southern outskirts.[4]: 470 Grosvenor Atterbury, a renowned architect, was commissioned to create Forest Hills Gardens.
The Long Island Rail Road opened a station at Forest Hills in 1911, and the Queens Boulevard trolley line followed two years later. The LIRR station was built with a brick courtyard, a clock tower, and arch-filled underpasses to match the Forest Hills Gardens area of the neighborhood.
